Cancellations
If you are going to miss a scheduled appointment you will need to call at least 24 hours in advance. Otherwise, you will be charged a $35.00 missed appointment fee that is not billable to insurance.
Emergency Cases
After Hours
If you are not a physician, this service is for our established patients only who recently had an in-office procedure.
Other Medical Issues
If you have any other medical emergency, you must call 911 or visit your nearest emergency room.
Payment / Insurance
We are an in-network provider for the majority of insurance plans in Arizona, including Medicare, Medicaid, and AHCCCS. Our office staff will be happy to help confirm if our practice accepts your insurance provider.
Patients are responsible for deductibles, co-payments, and remaining balances.
Medical insurance plans usually do not cover cosmetic procedures, and payment is due at the time of service. For your convenience, we accept cash and personal checks, as well as the following credit cards: Visa, Mastercard, Discover, and American Express.
